This is a 2 ply, or double layer baby receiving style blanket by The Childrens Place.

 It is tan/cream striped on one side and cream on the other with an elephant applique. The striped side has a tiny satin tag with an elephant in one corner. 

It has the usual double-layer wash result where the two layers do not perfectly line up anymore.
